Next Steps

Step 1: In-Depth Application

After your initial application is reviewed and accepted, you will be sent a link to complete the in-depth application through Docusign. Please make sure you answer every question to the best of your ability. As always, all of your information is confidential, and will only be used to determine the program that’s the best fit for you. We also encourage you to also start working on your medical documentation at this time. Once your in-depth application is completed, please give our team some time to review it and we’ll reach out to schedule your phone consultation! If we require additional information, we will let you know!

Step 2: Medical Information

We will ask you to provide us with all relevant medical documentation from medical provider. All medical information will be stored securely and be kept confidential. This information will be used by our trainers to understand how to teach and train your dog based on the the needs and limitations of the individual.

Step 3: Phone Consultations

After your application has been reviewed we will reach out for more specific information, this will help us when we set up your FREE consultation. This phone call will be with one of our trainers where you will discuss program details. During this call our trainer will also ask you questions so we can learn about your daily life, learn what you are looking for, and have a comprehensive understanding of your needs. At the end of the call, you will be provided time to ask us any questions that you may have.

If a client is over the age of 18 and would like to invite any family and/or friends to join them on any of these calls/ meetings, you are more than welcome to do so! All minors are required to have a parent/guardian included in all communication with CSD, including these call/meetings. Minors are also welcomed to invite additional family members and/or friends to join on them on these calls/meetings.

If you are interested in learning more of the costs, we are also happy to set up a call and answer all questions after your call with the trainer. During the money chat, you will learn about all non-training costs that you should be planning for. You will also be provided plenty of time to ask any questions about our program.

Step 4: Signing your Program Agreement and Pay Invoice

Once all details have been confirmed, we will send you your personalized program agreement via email and a link to pay for your program. The agreement will include confirmation of your total program cost and what’s included in your program. No services will start until the agreement is signed and your program has been paid for.

If there is an existing waitlist, you will be notified prior to signing the agreement and be provided an anticipated start date. Please be aware that delays in signing or payment may lead to your program’s start date being delayed as well. Additionally, your anticipated start date is subject to change.
